A sad lesson to be learned

September 21: Amercan Airlines flight 965 crashes in Columbia, December 1995. On September 17, 1996 The New York Times reports that a programing error may have been responsible for the crash. The error was one of information display and habit. Doing a task so many times that it becomes habit creates a problem. Displaying data in an odd way also creates a problem. The lesson learned is to review data displays and review some more. Also to never let any operation become so routine you don’t think about what your doing.
